CARE TEAM NURSE (RN/LPN)
Northwest Human Services is a non-profit leader in providing advocacy, quality healthcare and social services in Oregon since 1970.
We are currently seeking a Care Team Nurse (RN/LPN) to join our West Salem Clinic!
*YOUR ROLE AT NWHS*
As our Care Team Nurse, you will use a trauma-informed and whole-person approach to address patient wellness, acute illness, chronic disease, disability, and end-of-life needs in collaboration with their assigned primary care provider and care team. In this role, you will educate and motivate patients in areas related to self-care; dispense telephone advice to patients with minor acute problems; support the ongoing care management of patients with stable chronic disease; monitor patients during routine office visits; and perform of a variety of procedures that are within the scope of the professional nursing practice.
*HOURS & LOCATION*
This position is full-time 40 hours per week Monday – Friday; with rotating Saturday shifts.
MINIMUM QUALIFICATIONS
Current Oregon Nursing License (RN or LPN) is required
CPR/BLS certification is required
The ideal candidate will possess clinical expertise in an outpatient environment and triage.
Ability to exercise independent judgement and decision-making.
Ability to work well in a group setting.
Ability to prioritize competing responsibilities while under pressure.
Demonstrates problem solving skills.
Personal flexibility to perform in an ever-changing work environment with frequent interruptions.
Ability to manage high volumes of patients in a short span of time, while dealing with unpredictable issues.
Ability to deal with the public politely, courteously, efficiently, and effectively.
Ability to work with diverse cultures and lifestyles.
PREFERRED QUALIFICATIONS
Bilingual English/Spanish preferred
